What sustained output can it actually hold?: Around 750 lumens after the initial step-down. The headline figure describes the first half-minute; the 750 is what you'll be using on any real task.
Can it run 18650 cells?: Yes, with the included adapter sleeve — but the sleeve is loose enough to move around. Owners pad it with a wrap of paper. A 21700 is the better cell anyway.
Is it worth modifying?: It's one of the more commonly modified budget lights precisely because the host is good and the driver isn't. If you don't want to open a light up, that same fact means you're buying its main limitation.
